Cache-Control: no-cache or max-age=0

这个问题，很多文档都没有说清楚。

还是在 rack-cache 的配置说明里顺便给解释了。

no-cache 相当于 按住 control 点 refresh，硬刷。

max-age=0 相当于点 refresh，只是 validate，如果 stale 了再传输。
